This office is seven blocks directly east of the center of downtown Salt Lake City (Temple Square) on the south-east corner of Seventh East and South Temple. It is within one mile of both LDS Hospital and Salt Lake Regional Medical Center. There is ample off street parking east of the building. From Interstate 15 take the exit (Exit 370) to Sixth (600) South going east to Seventh where you turn left (north) and proceed six blocks. From Interstate 80 take the Seventh East exit (Exit125) going north and proceed 24 blocks.

Our office is on Sidewinder Drive in the Prospector Area of Park City. Sidewinder is one block east of the light at the corner of Bonanza/Monitor Drives and Kearns Road (Highway 248). We are just west of the Marriott Olympia Park Hotel. Eventually across the street from the office will be the Park City Medical and Surgical Center where we will have facilities for outpatient surgical procedures. HealthSouth which is building the facility and will manage it, has run into unexpected construction delays but expects to have the doors open by May, 1999. When the surgery center is open, it will be possible to perform virtually all aesthetic plastic surgical procedures right in Park City, eliminating the necessity of driving to Salt Lake City for surgical care. In the mean time we will continue to schedule procedures at the various outpatient facilities in Salt Lake, including the HealthSouth Salt Lake Surgical Center at 603 East 39th South, the Intermountain Surgery Center close to LDS Hospital, the Wasatch Clinics at the University of Utah on Fifth Southand Foothill Drive, and Bonneville Surgery Center at 1230 East 39th South. Services offered at the Park City office and not available at the Salt Lake City office are computer imaging to aid in visualizing post operative results, subdermal therapy using the ESC SilkLight device for reduction of skin texture problems such as cellulite, and tradition massage therapy. Our aestheticians, Traci and Misha, also have their principal treatment facility in our office.

Insurance Participation
Insurance programs change so frequently that this list may not be complete at this particular time. It is best to check with Heather, Shannon, or Felicia at the Salt Lake City office to be sure (801) 364-1201. Virtually all surgeries to be covered by insurance will be prior approved by our office staff with your insurance carrier at the time your surgery is scheduled.

It is necessary in all cases that fees for cosmetic be paid one week in advance of surgery date. In addition to cash and personal checks, the following financing options are available: Credit Cards MasterCard, Visa, American Express, Discover AMS Loan Program We can assist you in making application to the AMS Patient Financing Program, jointly sponsored by our office, The American Society of Plastic and Reconstructive Surgeons (ASPRS), and MBNA America Bank, N.A. By calling a toll free number and giving AMS our office's priority code, you can receive a loan approval decision in thirty minutes or less!
