Our office is directly east of the LDS Hospital in the Avenues neighborhood of Salt Lake City. We are on the second floor of the Intermountain Surgery Center Building, Suite 202, also identified as Building One. There is off street parking on two levels off of D Street just north of our office. . To reach our office from Interstate 15 take exit 370 to Sixth (600) South going east to Fifth East; turn left (north) and proceed six blocks to South Temple. The street jogs first to the left on South Temple then immediately right on E Street. Proceed eight blocks to Eighth Avenue; turn left. Then in one block turn right on D street where Eighth Avenue dead ends into the LDS Hospital Emergency Room. We are in the building on the north east corner of Eighth Avenue and D Street. From Interstate 80 take the Seventh East exit (Exit125) going north and proceed 24 blocks to Sourth Temple. Turn left and go three blocks to E Street where you turn right and proceed as described above.

In addition to all of the diagnostic and therapeutic resources of LDS Hospital, this office features an in office operating room for minor procedures under local anesthesia and procedures under intravenous sedation.
